Registration Cancellation Policy

Payment in U.S. funds MUST accompany the registration form in order to process the registration. To avoid duplicate charges, do not 
both fax and mail your form. Major credit cards, checks and purchase orders are accepted. Checks should be made payable to LMF.

All cancellations will be subject to a $20 administrative fee. Cancellations must be sent in writing toLMF Registration Center via fax or email. Telephone cancellations will not be accepted.

All cancellations must be received in writing by May 1, 2010. No refunds will be processed after this date. All refund requests 
received before May 1, 2010, will be processed after the conference. After May 1, 2010 substitutions will be allowed in the event the 
registrant is unable to attend, but no refunds will be issued. Telephone substitutions will be permitted.
